Properties
HYGROTON increases the excretion of water and NaCl without interfering with blood circulation. Its diuretic effect sets in after 2 hours, reaches its maximum after 12 hours and persists for about 2 days. It gently reduces elevated blood pressure even when administered in low dosages. This effect increases gradually within 2 – 4 weeks. It maintains its efficiency even if used for long periods. It may be combined with other antihypertensives. It is well tolerated.

Indications
Hypertension, oedema of cardiac, renal and hepatic origin. Premenstrual and idiopathic forms of oedema.

Dosage
To be taken preferably in the morning with breakfast. The aim should be to employ the lowest possible maintenance dose on an individual basis.
Hypertension: Adults: start with 25 mg daily. Should the effect be insufficient, the dosage can be raised to 50 mg daily and/or combined with other antihypertensives.
Oedema: Adults: start with 100 – 200 mg every other day or 100 – 200 mg daily in severe cases. After a few days reduce the dose to 25 – 50 mg daily (or 50 – 100 mg 3 times a week). Children: dosage should be adapted to the clinical picture. The average may go up to 2 mg/kg daily.

Side Effects
More common: Electrolyte and metabolic disorders such as hypokalaemia, hyponatraemia, hyperuricaemia, hyperglycemia, worsening of diabetes, and gout.
Less common: Skin rashes and photosensitisation. Postural hypotension and cardiac arrhythmia. Dizziness, paraesthesia and headache. Gastrointestinal disturbances, blood dyscrasias, impotence, disturbances of vision and intrahepatic cholestasis or jaundice.
Isolated cases: Pancreatitis, idiosyncratic pulmonary oedema, allergic interstitial nephritis and vasculitis.

Contraindications
Hypersensitivity to Chlortalidone, severe renal or hepatic failure, anuria.

Precautions
Disturbances in the serum electrolyte balance may occur during prolonged treatment, excessively strict low-salt diet should be avoided and high potassium diet is recommended. If additional loss of potassium occurs (vomiting, diarrhoea, rephrosis, liver cirrhosis…), potassium medication will need to be given under surveillance.

Pregnancy & Lactation
During lactation and the first trimester of pregnancy, HYGROTON should only be employed for compelling reasons.
