PROPERTIES
Indomethacin is a prostaglandin synthetase inhibiting agent. It has a marked analgesic, antipyretic and anti-inflammatory properties.
PHARMACOKINETICS
Indomode capsules are readily absorbed from the gastro-intestinal tract, peak plasma concentrations are reached about 2 hours after a dose.
The bioavailability of Indomode suppositories in adults is comparable with oral dosage forms.
The formulation of Indomode retard was developed with the aim of achieving a long duration of action and maintaining of therapeutic effect for a long period and decreasing side effects by decreasing the drug release in plasma and maintaining the minimum therapeutic concentration level.
INDICATIONS
The three dosage forms of Indomode are indicated in the active stages of:
• Rheumatoid arthritis.
• Moderate to severe juvenile rheumatoid arthritis.
• Osteoarthritis.
• Ankylosing spondilitis.
• Gout.
• Acute musculoskeletal disorders, such as bursitis, tendonitis, synovitis, tenosynovitis.
• Acute lumbago and chronic lumbago (blockage of the sacro-iliacal joint).
• Non-bacterial inflammation, pain and inflammation (trismus and oedemas) following dental procedures and orthopedic procedures associated with reduction and immobilization of fractures or dislocations.
• Pain associated with primary dysmenorrhea.
CONTRAINDICATIONS
Indomode should not be used in patients hypersensitive to indomethacin, in patients in whom acute asthmatic, urticaria, or rhinitis are precipitated by aspirin or other NSAIDs.
In pregnancy and during lactation.
It is not advisable to administer to children under 2 years of age.
Peptic ulcer and gastrointestinal lesions.
Suppositories are contraindicated in patients with a history of proctitis or recent rectal bleeding.
PRECAUTIONS
Indomethacin should be administered with caution in patients with impaired renal or hepatic function, diabetes mellitus, and to those with bleeding disorders, epilepsy, parkinsonism or psychiatric disorders. It should be used with greater care in the elderly.
INTERACTIONS
Indomethacin should not be co-administered with NSAIDs, diflunisal, and triamterene. It should be used with caution in patients receiving:
Probenecid – beta blockers – captopril – digoxine – gentamycin – haloperidol – hydralazine – methotrexate – anticoagulants – lithium.
SIDE EFFECTS
Most common side-effects are gastrointestinal disturbances, which may be reduced considerably with Indomode Supp. And Indomode Retard capsules. Effects on central nervous system include: headache, dizziness, depression, drowsiness, confusion, lightheadedness, insomnia and psychiatric disturbances.
Acute asthmatic crises may occur in aspirin sensitive patients or with history of asthma.
Burning, pain, discomfort and itching may occur with use of indomethacin suppositories.
DOSAGE
1 – 2 suppositories daily.
2 – 4 Caps. daily administered with milk or antacid.
1 – 2 retard Caps. daily.
In certain conditions it is recommended to administer 1 supp. at bedtime to be supplemented with capsules during the next day.
