Metronidazole

Composition
Oral suspension: each 5 ml contains the equivalent of 125 mg metronidazole.
Tablets: each tablet contains 250 or 500 mg metronidazole.
Indications
Metronidazole is active against protozoa and anaerobic bacteria. It has a wide range of uses including ameobiasis, giardiasis, trichomoniasis vaginosis. It is used in inflammatory bowel disease, acne rosacea and peptic ulcer therapy against helicobacter pylori in combination with bismuth subsalicylate and tetracycline, or chlarithromycin and omeprazol.
Side Effects
The side effects of metronidazole are generally dose related.
The most common are gastro-intestinal disturbances. Neurological disturbances, headache, weakness, dizziness, drowsiness, insomnia may occur.
Administration and Dosage
The tablets are taken with or after food. But the suspension at least one hour before food.

Intestinal and hepatic amoebiasis:
Adults: 1.5 – 2 gr daily.
Children: 40 mg/kg body weight daily, in divided doses 2
to 3 times daily for 5 – 7 days.
- Giardiasis:
Adults: 250 or 500 mg 3 times daily for 5 – 7 days.
Children: 15 mg / kg body weight for 5 days in divided
doses.
A second course may be given after an interval of 10
days.
- Urogenital trichomoniasis, bacterial vaginosis:
250 mg 3 times daily or 500 mg daily, for 7 days, to be
repeated after 7 days, the male and the female should be
treated concomitantly.
- Anaerobic bacterial infections:
Initial dose 2x500 mg followed by 500 mg every 8 hours
for 7 days.

Treatment of acne rosacea:
250 mg twice daily depending on severity of the disease.

Helicobacter pylori:
In combination with tetracycline and bismuth subsalicylate, 500 mg 3 times daily for 14 days. In combination with chlarithromicyn and omeprazol, 500mg twice daily for one week.
Contraindications
Lactation and pregnancy especially the first 3 months.
It is advisable to abstain from alcohol during treatment.
Precautions
Metronidazole should be used with great care in patients with active disease of the central nervous system.
Drug Interactions
Metronidazole may enhance the anticoagulant effect of warfarin, and may impair the clearance of phenytoin, lithium and fluorouracil.
Cimetidine may increase the concentration of metronidazole in the plasma.
Phenytoin may accelerate metronidazole metabolism.
